


All Title V permits have a standard permit condition that requires semi-annual reports of all required monitoring. The District has prepared an advisory to help facilities comply with this requirement. The monitoring reports should follow the parameters listed below:
- The first report is due six months after the permit is issued.
- All instances of non-compliance shall be clearly identified in these reports.
- The reports shall be certified by the responsible official as true, accurate, and complete.
- If a facility is only required to submit a semi-annual report because of the standard condition in Section I of its permit, the report should simply summarize any monitoring required by specific permit conditions or applicable regulations.
- If no monitoring was required during the reporting period, the report should state that no monitoring was required.
Also:
- All instances of non-compliance with the permit shall be reported in writing within 10 calendar days of the discovery of the incident.
- Within 30 calendar days of the discovery of any incident of non-compliance, the facility shall submit a written report including the probable cause of non-compliance and any corrective or preventive actions.
